13.2 Outcomes Framework for Residential Placements - Guidance for Completion

The Outcomes Framework is for all residential placements where Nottinghamshire children are placed.

1. Aims

  • To evidence outcomes for young people in placement;
  • To be able to measure change and progress for young people;
  • To be able to target / identify key areas for placement providers to work on with the young people;
  • To improve matching process;
  • To be able to identify strengths of particular placements;
  • To identify inhibitors as well as progress.


2. Principles

8 Key Areas, sub-divided into Target Areas (see the Outcomes Framework Process diagram).

Each Target Area has its own Outcome Indicators, against which the young person can be measured. Example:

Key Area: Physical Health
Target Area: Medical Health
Outcome Indicator: Young person has a good standard of dental and optical health


3. Responsibilities

Provider Role/ Key Worker

  • Lead responsibility for completing Baseline Form;
  • Complete Baseline form (with input from social worker, and young person) for 72 Hour/ Placement Planning Meeting. Informs Care Planning;
  • Address each Outcome Indicator
    Apply R/A/G rating to identify key areas
    Set targets in order to progress towards improved outcomes;
  • Baseline form verified by young person, IRO. Signed off by social worker at first review;
  • Send completed form to Placements team (copy to social worker) after first Review /within 28 days, whichever is soonest;
  • Lead responsibility for completing Review form prior to LAC Reviews, with input from young person, social worker and other relevant agencies;
  • Provide specific examples and evidence of how progress has been made, including R/A/G rating;
  • Circulate copy of Review form prior to review to IRO for the child / young person /social worker;
  • After Review, send verified Review form to Placements team (copy to social worker), within 5 days;
  • Repeat process for subsequent reviews. New targets and outcome indicators can be set following each Review.

Social worker Role

  • Work with Provider and young person to provide information to complete Baseline form for Placement Planning Meeting, including setting targets and identifying desired outcomes;
  • Sign-off completed Baseline form at first Review;
  • Upload Baseline form to Framework within 5 days of Review;
  • Work with Provider and young person to provide information for Review form;
  • Verify / challenge evidence examples;
  • Sign-off completed Review form at Review;
  • Upload Review form to Framework within 5 days of each Review;
  • Repeat process for subsequent reviews. New targets and outcome indicators can be set following each Review.

Young Person

  • Participate and contribute to setting of targets, and gathering of evidence.

IRO’s

  • Aware of tool as additional source of information Provider will present at Reviews;
  • To be provided with completed Baseline / Review forms prior to reviews, by the provider;
  • To verify Baseline / Review forms, in agreement with social worker;
  • To alert Placements Team / Service Manager if forms not being presented at Reviews.

Placements Role

  • Administer and monitor Outcomes Framework;
  • Send Outcomes pack to provider when placement confirmed, as part of contract with IPA;
  • Ensure provider understands expectations regarding the Outcomes Framework;
  • Receive completed copy of Baseline form from provider after first LAC review or within 28 days, whichever is soonest;
  • Receive completed copy of Review form after each LAC Review from provider;
  • Collate the information and has overview of progress;
  • Concerns re lack of progress to be raised with provider (and with social worker / IRO as necessary);
  • Use Evidence Example Guidance to assist in assessment of provider quality.
